A complete and updated immunization record is required for individuals 18 years and under, who have an appointment in our immunization clinic. You may bring the updated immunization record to the office or for a shorter wait-time on the day of your appointment, you may upload your immunization record using our secure site.

If you do not have an updated immunization record, a copy can be obtained from your pediatrician, the school nurse, or the last place vaccines were received.
